  • MEDICAL, DENTAL & VISION

    Memorial offers two medical coverage options so you can select the best plan for you and your family. The two plans available are the Premium Plan (PPO) and a Consumer-Driven Health Plan (CDHP). Both medical plans provide the same high level of benefits, including full preventive care coverage. Cost-sharing amounts like deductibles, copays, out-of-pocket maximums and colleague premiums vary depending on the plan and number of dependents covered. Colleagues who work 32 hours per 2 week pay period are eligible for coverage. Every new colleague meets one-on-one with a benefits expert to choose the options that are best for your specific situation.

    Memorial Choice is part of either medical plan. Memorial Choice offers a variety of services at Memorial Physician Services, SIU and Springfield Clinic with no copay or coinsurance.

    Healthy teeth are a part of overall wellness. Two dental plan options for you and your family are available, one with orthodontic coverage.  The vision plan offers the opportunity to purchase coverage for you and your family at a low biweekly rate. Coverage includes yearly exams and contact lenses and/or lenses and frames.

  • FLEXIBLE SPENDING ACCOUNT

    Memorial colleagues can elect two types of FSAs—a healthcare FSA and dependent care FSA. The healthcare FSA allows you to set aside tax-free dollars each year to pay for certain eligible medical, prescription drugs, dental, or vision expenses. The dependent care FSA allows you to contribute pre-tax dollars to pay for eligible dependent daycare expenses. FSAs are available to all benefit-eligible colleagues.

    Memorial Health System has partnered with Fidelity to help manage retirement savings.

  • LIFE INSURANCE & VOLUNTARY BENEFITS

    Memorial provides basic life insurance at no cost to you to provide financial protection in the event of your death. Options also exist for you to purchase additional coverage for you, your spouse and dependent children at competitive group rates.

    A suite of voluntary benefits are available to provide additional financial security. You can choose from whole life insurance, group accident and critical illness insurance with coverage available for you, your spouse and children.

  • EMPLOYEE ASSISTANCE PROGRAM & COLLEAGUE WELLNESS

    Every Memorial colleague has access to the Employee Assistance Program through Memorial’s Colleague Assistance Services to resolve a wide range of personal problems. Resources include marital and family counseling, crisis interventions, alcohol/drug counseling, eldercare support, financial counseling, identify theft support and legal issues. All interactions are kept confidential and are free to colleagues and their dependents.

    Health for Life is designed to give colleagues resources and tools to help with disease prevention, reducing health risks and continuing their health and well-being journey. Health for Life includes the opportunity to participate in a confidential online health assessment, optional wellness coaching and a free tobacco cessation program, as well as access to online resources that support your desire to lead a healthy, happy life. Additionally, Memorial’s Colleague Health and Wellness department provides annual flu vaccines for no charge to all health system colleagues and year-round health counseling.

  • OTHER BENEFITS

    Adoption Assistance

    Memorial wants to support colleagues in balancing their workplace demands with their personal and family needs, and to do so, we provide generous paid time off and leave policies. We also recognize the particular investment of time and financial resources normally necessitated by the adoption process. As a result, Memorial has established an Adoption Assistance Plan to provide financial assistance for adoption-related expenses.
