Welcome to the Memorial Health System Foundation Healthcare Career Education Grant Application!

Completed application with all requested information must be submitted by 4:00pm CST on June 1st annually in order to be considered for a foundation education assistance opportunity.

All education grants are awarded based on availability of funds and alignment with MHS strategic workforce needs.

Payment, Work Commitments and Tax Implications

  • Foundation Education Grant Disbursements
    • Foundation Healthcare Career Education Grants are not paid in lump sum amounts. Payments are made over the course of the degree program.
    • Foundation Healthcare Career Education Grants disbursements are processed bi-weekly and will be issued via check through Accounts Payable. Recipient should allow at least two weeks for processing.
    • Recipients must furnish grade reports to the Organization Development department via the TuitionManager portal in alignment with the disbursement schedule that will be determined during the contract review meeting.
    • In order to receive disbursement the following eligibility must be met:
      • Student must maintain full-time or part-time academic status.
      • Student must maintain a 3.0 grade point average on a 4.0 scale each semester. If student fails to meet this requirement, their next scheduled disbursement will be forfeited.
        • Student will then have a probation period of one semester to achieve the GPA requirement. If they achieve the 3.0 the next semester, their disbursement schedule will begin with the next scheduled disbursement. (Please note, the forfeited disbursement cannot be recovered and the total amount of the grant will be adjusted to reflect the forfeiture.)
        • If minimum GPA is not achieved after one semester suspension of funding, the student will be in default on the grant agreement and will be contacted by Organization Development to create repayment plan for any monies previously disbursed.
  • Foundation Education Grant Work Commitment
    •  To receive grant funding, recipients must sign a contract agreeing to a work commitment with that foundation’s associated hospital upon the completion of their course of study.
    • The work commitment will begin at the completion of the recipient’s course of study and receipt of license (if hired into a licensed role). The work commitment is as follows:
      •  $5,250.00 received = 1 year to a full-time position (2,080 hours of work); with a minimum of a one year work commitment for any award amount received.
      • For each additional $5,250.00, an additional year (2,080 hours of work) is required, up to a maximum of five years (10,400 hours of work).
  • Repayment
    • Recipient shall reimburse Memorial Health System the full amount of the grant paid pursuant to the education agreement in the event:
      • Recipient fails to complete all of the necessary requirements to graduate from the degree program.
      • Recipient fails to obtain degree.
      • Recipient fails to work for Memorial Health System as required by the education grant agreement
      • Recipient’s employment at Memorial Health System is terminated prior to completing the work commitment as the result of student’s willful misconduct, for performance- related reasons or behavior not consistent with the Mission and Vision and Statement of Values of Memorial Health System, or due to student’s failure to maintain a license required for employment (as determined by Memorial Health System).
      • Recipient voluntarily resigns from employment at Memorial Health System while still a student or prior to completing the required work commitment.
    • Recipient hereby agrees that Memorial Health System may deduct any amounts owed to pursuant hereunder from any wage or salary payment due to colleague. Memorial Health System hereby reserves the right to apply interest on any unpaid principal balance of colleague’s repayment obligation at an annual rate of ten percent (10%).
  • Foundation Education Grant Tax Implications

    Applicant acknowledges and understands that receipt of a Foundation Healthcare Career Education Grant may be taxable income to the student depending upon current tax laws, and student should consult with their tax advisor regarding income tax implications of a Foundation Healthcare Career Education Grant and how it may affect their personal situation now or in the future based upon current and future tax laws which could govern the tax treatment of amounts covered under a Foundation Education Grant.

Guidelines, Eligibility and Application Process

  • Guidelines for Education Grants
    • There will be one application cycle per year.
    • Application Deadline: April 1 annually
    • Awards will be announced between May 1 and June 30.
    • Grants are awarded for programs that are identified as strategic workforce needs for Memorial Health System at each affiliate location.
    • Maximum award levels are contingent on availability of affiliate foundation funds.
  • Academic Coursework Availability
    • Applicant must be accepted into their program of study and all pre-requisite coursework must be completed.
    • Applicant’s selected education institution and program of study must both be fully accredited.
    • Must have a cumulative grade point average of at least a 3.0 on a 4.0 scale must have attained no lower than a “B” average in core classes completed at the time of application.
  • Application and Review Process

    Application Process 

    • Applicant will complete application.
    • Applicant will need to collect the following items before submitting an application:
      • Verifiable documentation of total program cost
      • Documentation of core classes in selected program of study with projected dates of enrollment
      • Acceptance Letter
      • Most recent transcript
      • State W4 form
      • Federal W4 form

    Review Process 

    • All applications will be reviewed by the appropriate affiliate foundation within 60 days of the April 1st deadline.
    • Decisions will be rendered based on the following criteria:
      • Education preparation will lead to a position that meets a business and/or strategic need of the organization.
      • Foundation funds available to support grant
      • The applicant meets all the eligibility requirements
    • Applicants will be notified of their acceptance or denial for a Foundation Healthcare Career Education Grant
